Job Description:
Patient Transfer Coordinator - North Highlands, CA
Patient Transfer Coordinators are responsible for receiving and
coordinating emergency and non-emergency patient transfer and
transport requests via air or ground ambulance. Patient Transfer
Coordinators will routinely interact with both external and
internal customers, including but not limited to: hospital
personnel, physicians, administrators, transport vendors and other
key personnel by performing the following duties.
Experience:
--- Preferred 1+ year prior experience in the health care
environment
--- Preferred 1+ year customer service experience
--- EMS dispatch experience preferred
--- Knowledge of medical terminology required
--- Computer based phone systems, VOIP/Digital
Duties:
Promotion to Coordinator I requires meeting all requirements of the
Patient Transfer Coordinator Trainee and successfully passing a
skills assessment, written evaluation, and oral board within 18
months of hire. Additional requirements include demonstrated
competency in the following areas:
- Focused interaction with singular clients and mental health
transfers
- Setting up transports
- Auto-launch or Auto-dispatch air or ground ambulances
- Knowledge of Levels of Service
- Knowledge of County EMS Regulations related to ambulance Levels
of Service
- Discharged patients
- Private residence
- Skilled Nursing
- Long Term Acute Care (LTAC)
- Insurance notifications
- Kaiser EPRP notifications
- Blue Shield Notifications
- Transport follow-ups for ground
- Bed Checks
- EMTALA Transfers (ED to ED)
- EAP
- Building patients from face sheets and using Client EMRs
- Increased Typing Speed of 40/WPM net
- Demonstrated working knowledge of LVM software
-Proficiency using clients Electronic Medical Record programs, such
as Cerner, Epic and MediTech
- Working knowledge and proficiency in following call algorithms
for each client.
- CQI and Close calls with a > 90% accuracy rate
- Setting Transport
- Auto launch categories and process
- Difference in ground and sir levels of service
- Transport follow-ups
- EMSystems
- Pulling face sheets and faxing records
- Bed Checks
- Discharge patients' home and transport arrangements
- Insurance - Kaiser first call notifications and contact with
EPRP
- Insurance - Blue Shield first call notifications
o Regulation and Patient Flow process
- Client flow charts
- EMTALA
- HIPAA
Education:
--- High School diploma or equivalent.
--- Two-year degree in related field preferred
Skills:
Credentials/Licenses:
--- Current or previously held EMT certification, Medical
Assistant, Case Manager or equivalent education/experience
preferred
--- Must successfully pass all tests associated with the PTRC
internal training program.

correspondence and articles using original or innovative techniques
and /or style.
Mathematical Skills:
Must be able to work with basic mathematical concepts such as
addition, subtraction, multiplication and division and to apply
concepts such as fractions, percentages, ratios and proportions to
practical situations.
Reasoning Ability:
Must be able to deal with and resolve problems; foresee and correct
potential issues without direct input or supervision. Ability to
apply common sense, sound judgment and the understanding to carry
out detailed written or oral instructions. Required the ability to
solve practical problems and deal with a variety of concrete
variables in situations where only limited standardization exists.
Ability to interpret a variety of instructions furnished in
written, oral, diagram, or schedule form. Ability to apply
principles of logical or scientific thinking to a wide range of
intellectual and practical problems. Ability to deal with nonverbal
symbolism in its most difficult phases. Ability to deal with a
variety of abstract and concrete variables.
