The Patient Transfer Coordinator is responsible for receiving and
coordinating emergency and non-emergency patient transfer and
transport requests via air or ground ambulance at various levels of
complexity. You will routinely interact with both external and
internal customers, including but not limited to: hospital
personnel, physicians, administrators, transport vendors and other
key personnel. All Patient Transfer Coordinators begin as trainees
and are promoted at regular intervals based on successfully passing
a skills assessment, written evaluation, and oral board within 18
months of hire.ESSENTIAL FUNCTIONS/DUTIES
- Focused interaction with singular clients and mental health
transfers
- Setting up transports which may include both air and ground
assets
- Coordinate relevant transport modes for discharged
patients
- Perform transport follow-ups for ground
- Must demonstrate Technical Skills including following:
- Typing - Minimum 35/WPM net, increasing to Typing Speed of
40/WPM ne
- Demonstrated working knowledge of LVM software
- Proficiency using clients Electronic Medical Record programs,
such as Cerner, Epic and MediTech
- Working knowledge and proficiency in following call algorithms
for each client.
- Proficiency in telecom systems, conferencing multiple parties
and use of ARC software.
- Continuous Quality Improvement, closing calls with a > 90%
accuracy rate
- Proficiency in setting patient transports and follow-ups using
the appropriate levels of service and insurance protocols.
- Regulation and Patient Flow process, Client flow charts,
EMTALA, HIPAA, CAD Documentation
- Call Intake process and documentation, Call Flow, correct
algorithm used
- Knowledge of emergency procedures and Emergency Action
Plan/Post Event/Incident Plan
- ICS Basic Credentialing (ICS 100, 200) QUALIFICATIONS
Experience:
- Excellent communication skills and customer service skills
- 1+ year prior relevant experience in the health care preferred
but not required, e.g., Medical Assisting, Case Worker, EMT EMS or
Law Enforcement Dispatch or related discipline
- 1+ year customer service experience, insurance, or
billing/coding environment (preferred)
- Basic Knowledge of medical terminology required
- Ability to competently use computer based phone systems,
VOIP/Digital Education:
- High School diploma or equivalent.
- Two-year degree in related field, preferred but mot required
Credentials/Licenses:
- Current or previously held EMT certification, Medical
Assistant, Case Manager or equivalent education/experience
preferred
- Must successfully pass all tests associated with the PTRC
internal training program. Interpersonal Skills:
- Able to maintain professionalism and effectiveness in high
stress situations
- Must be able to work independently with little to no
supervision
- Must be able to work as an integral part of a cohesive, fast
paced and productive team
- Excellent oral and written communications
- Critical listening skills
- Problem solving and conflict resolution REPORTING
