We're also certified by the Joint Commission as a Primary Care Medical Home and a Behavioral Health Home.Position SummaryThe Dietician provides medical nutrition therapy to patients in one-on-one and group settings through the assessment of nutritional status, education, setting and monitoring of behavioral change goals. The Dietician is a member of the Sweet Success multidisciplinary team interfacing with the patient and other healthcare providers involved in the patient's care. The Dietician demonstrates competency in providing care for women with diabetes and pregnancy.Required Qualifications Bachelor's degree in Nutrition, Dietetics or related field.At least 1 (One) year previous experience in a perinatal diabetic clinic required.Experience in outpatient diabetes management; in the care of obstetrical patients OR an equivalent combination of education and job experience sufficient to successfully perform the essential duties of the job.Exceptional interpersonal skills.Excellent customer service experience including active listening, problem solving, and the ability to remain calm in emotional or stressful situations.Fluency in additional languages preferred.Computer (PC) literate in general office functions such as MS word, access, email and preparation of spreadsheets. NextGen experience a plus.Willing to work well with others and be a team player.Enjoy working with pregnant women.Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills.Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.Assist and support the Center/Department/Program to meet standards of High Reliability.Be willing to travel to multiple locations in Sacramento area.Must have valid driver's license and insurable driving record.Essential Responsibilities Conduct individualized nutrition needs/assessments for patients with diabetes and pregnancy.Plan and provide individualized and group instruction in nutrition to patients with diabetes and pregnancy, their families and significant others.Provide follow-up and monitoring of nutritional plans.Reviewing blood Sugar, teach diabetes class, assessing need for medical nutrition therapy, physical activity and medication.Initiating medication such as insulin under MD supervision and adjusting medication as needed to manage blood glucose levels.Educating patients on the administration of medications and identify/treat side effects. The employee may experience a number of unpleasant sensory demands associated with the client's use of alcohol and drugs, and the lack of personal care.
