hospital experience preferred Description: The RN Case Manager will
demonstrate resourcefulness, superior written and oral
communication, diplomacy, and organizational and analytical skills.
This role requires self-direction, assertiveness, and creativity in
problem-solving, systems planning, and patient care management in a
high-volume work environment. The RN will be required to float to
other facilities as needed. The patient population includes adults
and geriatrics, with a daily census of approximately 120. The unit
has 200 beds and offers interdisciplinary support, including
interpretation services, social services, and transportation. The
RN will be involved in care coordination and discharge planning.
The role requires an understanding of needs assessment, ordering
durable medical equipment, and developing a plan of care. Knowledge
of CMS guidelines, HIPAA regulations, and The Joint Commission
standards is essential. Candidates should possess excellent skills
in care coordination and discharge planning to effectively manage
