Creek, California, 94595! We are seeking an experienced Travel
Registered Nurse (RN) – Case Manager to join a fast-paced acute
care hospital team. The RN Case Manager will coordinate
comprehensive patient care plans, facilitate safe and timely
discharges, and collaborate closely with physicians and
interdisciplinary teams to ensure optimal patient outcomes. The
ideal candidate is highly organized, patient-focused, and
comfortable managing multiple cases while maintaining compliance
with hospital and regulatory standards. This role plays a critical
part in improving care transitions, reducing length of stay, and
supporting quality patient care delivery. Key Responsibilities:
Coordinate and manage individualized patient care plans Facilitate
discharge planning and care transitions Collaborate with
