seeking an experienced Registered Nurse (RN) with a strong
background in Home Health Case Management to join our team. This
position is responsible for the comprehensive oversight of patient
care in the home health setting, including direct patient care and
case management responsibilities. The RN Case Manager ensures that
quality, cost-effective care is delivered in accordance with
current clinical standards, regulations, and best practices.
Required Qualifications: Active California RN License Current BLS
Certification (American Heart Association required) Minimum of 1
year of recent Home Health Case Management experience Preferred
